The phone rings at the office more about this than about almost anything else.
Somebody has read that sugar feeds cancer. They believe it, because it’s true. And then they look at the glass of carrot juice we’ve been recommending for thirty years — bright orange, unmistakably sweet — and they cannot make the two things sit together.
Usually the question comes out apologetically, as though they’re catching us in something. “I don’t want to be difficult, but isn’t carrot juice just sugar?”
It’s a good question. It’s the right question. And the answer is worth more than the thirty seconds it usually gets.
Start With What’s Actually True
Cancer cells do consume glucose at a remarkable rate. We’ve written about that separately — the PET scan works by injecting a sugar molecule and photographing where it lands, and the tumors light up because they’re pulling it in faster than anything around them.
So the instinct is a sound one. Watch the sugar.
But there’s a second fact that rarely travels alongside the first, and without it people end up in strange places.
Every cell in your body runs on glucose. Your brain, your heart, your immune system. You cannot starve a tumor by removing carbohydrate from your plate, because your body will make glucose out of protein and fat rather than let your blood sugar fall. What you can do is stop flooding the system — stop the spikes, stop the insulin surges, stop handing over the stripped-down, fiber-free version.
That is a different instruction from “avoid everything sweet.” And the difference is where carrot juice lives.
The Numbers Nobody Looks Up
Here’s what actually happens when you drink a glass of it.
Sugar. An eight-ounce glass of unsweetened carrot juice carries about nine grams of sugar. The same glass of fresh-squeezed orange juice carries twenty-one. Apple juice runs higher still. A cola has more than double the sugar of carrot juice, ounce for ounce.
Glycemic index. Fresh carrot juice measures 40 to 43. Anything under 55 is considered low. A raw carrot itself sits down at 16.
Glycemic load — which accounts for portion size and is the more honest measure — comes in between 2.6 and 9 per cup. Under 10 is low.
So the food that gets treated as a sugar bomb is, by the actual measurements, a low-glycemic food.
And then there’s the part that surprises people. In small clinical studies, drinking fresh carrot juice daily has been shown to lower fasting blood glucose. It went down, not up. The antioxidants in carrots protect the pancreatic cells that make insulin and improve how well the body responds to it.
We are not telling you sugar doesn’t matter. We’re telling you that “it tastes sweet” and “it behaves like sugar in your bloodstream” are two different claims, and only one of them is true here.
What Rides Along With It
This is the part that settles the question.
When you drink a soda, you get sugar and nothing else. When you drink carrot juice, the sugar arrives in company — and the company is remarkable.
Falcarinol. A compound concentrated in the core of the carrot. It’s strongly anti-inflammatory, and it works in a way researchers call biphasic: at low concentrations it supports normal cell health, at higher concentrations it’s toxic to cancer cells specifically. It shuts down inflammatory signals inside tumor tissue that healthy tissue doesn’t depend on. It’s also destroyed by heat — two minutes at cooking temperature and it’s gone. Which is a large part of why we press so hard on raw.

Falcarindiol. Falcarinol’s partner, concentrated in the peel. On its own it pushes cancer cells toward self-destruction. Together with falcarinol the two amplify each other dramatically against leukemia and colon cancer cells. Don’t peel your carrots. Scrub them.

Alpha-carotene. About a quarter of the carotenoids in a carrot, and here’s the surprise — it is a more potent cancer-preventive compound than beta-carotene, which gets all the attention. In animal work it suppresses tumor promotion in liver, lung and skin far more strongly. In human liver cancer cells it restricts the cell’s ability to migrate and invade.
Beta-carotene. Three-quarters of the carotenoid load. Converts to vitamin A, which the immune system cannot function without. It also improves communication between cells, which is one of the things that goes wrong first in cancer.
Lutein. A smaller share, highest in yellow carrots, and a strong protector of cell membranes against free radical damage.
None of that is in a doughnut.
Does It Hold Up in People?
This is where the carrot stops being folk wisdom.
A 2023 meta-analysis pulled together 198 observational studies, including 50 prospective cohorts covering 52,000 cancer cases. High carrot consumption tracked with a 10% lower overall cancer incidence. And when researchers measured alpha-carotene in the blood rather than asking people what they ate — a far more reliable measure — the association strengthened to a 20% reduction.
The Danish “Diet, Cancer and Health” study followed 55,756 people for more than twenty-five years. Eating more than about 32 grams of raw carrot a day — two to four carrots a week, which is nothing — was associated with a 24% reduction in lung cancer, and it moved in step with the dose. Reductions showed up in leukemia and colorectal cancer as well.
Here is the line in that study we want you to notice: cooked carrots showed no protective effect against those cancers. The heat had taken out the compounds doing the work. Same vegetable. Different outcome.
A 2017 meta-analysis of six studies found a 37% lower bladder cancer risk in the highest carrot intake group.
And on juicing specifically: a controlled crossover trial gave sixteen adults the identical quantity of beta-carotene, once as raw carrots and once as fresh carrot juice. The juice produced 2.33 times the peak blood concentration and 2.09 times the total absorption. Same nutrient, same amount, twice the delivery — because juicing breaks apart the fibrous matrix that was holding it in.
In a separate intervention, breast cancer survivors drinking eight ounces of fresh carrot juice daily showed a measurable drop in systemic oxidative stress.
Twenty-Five Years of People Doing It
Before the research, there was the practice. There’s a letter in our own archive that says it better than any study does.
Jim Breen was diagnosed with renal carcinoma in October 1999. He went home, started the Hallelujah Diet, and drank eight eight-ounce glasses of carrot juice a day. Then he called my father, and this is what he wrote about that conversation:
“Within a few days I was talking to Dr. Malkmus from Hallelujah Acres, and he said if he had an active cancer he would not eat fruit. He told me that the natural sugars in fruit might be helping spread my cancer. So I quit eating fruit for one meal and ate another salad instead.”
Fruit down. Carrot juice in. Same phone call, more than twenty-five years ago, and the shape of it is what we still tell people — though we’d put it less absolutely than he did. Pull the fruit back rather than cut it out, and keep it to the lower-sugar end.
That’s the part worth sitting with. Not one man’s outcome — his letter is in the testimonies, and you can read it for yourself. What matters is that people with active disease have been drinking six and eight glasses of carrot juice a day, for months and years, since before most of this research was published. If the sweetness in the glass were the problem, we would have seen it by now.
Ann Cameron’s Story
There is one account that comes up every time this subject does, and it deserves telling properly.
Ann Cameron — a children’s book author, and no relation to my wife Ann — had surgery for stage 3 colon cancer in June 2012. She declined chemotherapy and radiation, having watched her husband die of lung cancer in 2005 despite them. Six months after her surgery, a scan found the cancer had spread to both lungs. That made it stage 4.
She had read about a man named Ralph Cole who had used carrot juice for his own cancer. She began drinking the fresh juice of five pounds of carrots a day — roughly forty ounces, spread through the day on an empty stomach. She made no other significant change to how she ate.
Eight weeks later, a CT scan showed the lung tumors had stopped growing and her lymph nodes had started to shrink. At four months the tumors were regressing and her thoracic lymph nodes had returned to normal size. At eight months, the scans showed the stage 4 cancer was gone. Her only side effect was carotenemia — a faint orange cast to the skin, which is harmless and fades. She wrote a book about it in 2015.
We tell you this as what it is: one woman’s documented case, not a clinical trial. Nobody has run the trial. We are not going to dress a single case up as proof, and we’re not going to pretend it didn’t happen either. It happened, it’s documented, and it is the reason a great many people started juicing carrots.
Juice It or Blend It?
We owe you honesty on the one place where the evidence genuinely pulls in two directions.
For juicing: the absorption data above is real, and a glass of juice goes down when a person in treatment cannot face a meal. That matters more than it sounds.
For blending: you keep the fiber, which steadies blood sugar, feeds the gut bacteria that hold inflammation down, and helps with the constipation that comes with treatment. And some falcarinol stays behind in the pulp a juicer throws away.
Our standing practice is juice, and we’d hold to it — the bioavailability advantage is substantial and the point of juicing is to get deep nutrition into a body that may not be able to eat much. But if you’re well enough to eat, blending is not a lesser choice, and anyone who tells you the question is settled hasn’t looked at it carefully.
How We Do It
A few standards we’ve arrived at over many years, some of which run against what you’ll read elsewhere.

- Juice vegetables. Never fruit. Take the fiber out of fruit and you’re drinking concentrated sugar with nothing left to slow it — which is the one thing we’re trying to avoid. Eat fruit whole, in small portions, and less of it while disease is active.
- Scrub, don’t peel. The falcarindiol is in the skin.
- Carrot in moderation as part of a mix, not as the whole glass, unless you’re following a specific protocol with someone guiding you. Cucumber and celery for volume, collard and dandelion greens, a little ginger, lemon, parsley.
- Skip kale and spinach in the juicer — kale in daily quantity presses on thyroid function, and spinach carries oxalates that are hard on the kidneys when you drink them every day. Fine in food; not daily in juice.
- Drink it fresh — and mind the machine as much as the clock. Our own research director, Michael Donaldson, PhD, has measured this directly. In his household juicer comparison, press and twin-gear extractors preserved substantially more enzyme activity than centrifugal machines — the People’s Press ran 20% higher than the Norwalk and Green Life, 50% higher than the Omega, and two and a half times higher than the centrifugal Juiceman. The juicer you own does more damage in ten seconds than the refrigerator does in a day. In carrot juice held cold, enzyme activity fell only about 10% over 72 hours. What goes first is vitamin C, which drops on the order of 8 to 40% by day seven in the fridge and far faster on the counter. And past about the third day the real issue stops being nutrition and becomes bacterial growth. Make it fresh, keep it cold and covered in glass, and drink within the day.
Where We’d Be Careful
- If you smoke, or you’ve had asbestos exposure. Two large trials found that high-dose synthetic beta-carotene supplements — 30 mg a day in a capsule — raised lung cancer risk in smokers. That finding is about isolated supplements, not about carrots or carrot juice, and it’s worth being precise since it gets repeated carelessly. Get your carotenoids from food, not from a pill.
- If you have diabetes. The glycemic load is low, but juice has no fiber to slow it. Watch your numbers rather than assuming. A little good fat alongside the glass — a spoonful of flax oil, a few raw nuts or seeds, half an avocado — slows absorption and takes the edge off any rise. It is a simple fix and it works.
- If you have kidney disease or advanced liver disease. Forty ounces of carrot juice a day is a very large potassium load. This is a real limit, and one to work through with a nutritionally minded health care professional.
How much? What we typically suggest is one to two eight-ounce glasses of fresh vegetable juice a day — carrot as part of a mix rather than the whole glass — alongside two servings of BarleyMax. That puts the carrot portion itself at roughly four to five ounces a day, which is about right for general health. The therapeutic accounts — Ann Cameron’s among them — run to forty ounces daily from five pounds of carrots. That is a different undertaking, and not one to start without someone alongside you.
The Design, Again
Genesis tells us God gave us every seed-bearing plant for food. What strikes me about the carrot is how ordinary it is. It’s the cheapest thing in the produce section. It grows almost anywhere. It’s been in the ground under our feet the whole time, and it turns out to be carrying compounds that researchers are only now naming.

That seems to be the pattern. The provision is already there, in plain sight, at almost no cost, and we go looking somewhere else.
Sugar in a body fighting cancer is a real concern, and we hold that line hard. But the way to hold it is to look at what a food actually does in the body — not to flinch at anything that tastes sweet. A glass of carrot juice and a can of soda have almost nothing in common except that they’re both orange.
Keep the juicer on the counter.
Questions We Hear Most Often
Q: Does the sugar in carrot juice feed cancer?
A: Not in the way people fear. Carrot juice is a low-glycemic food — about nine grams of sugar per cup, against twenty-one in orange juice, with a glycemic load under 10. More to the point, the sugar arrives with falcarinol, falcarindiol, alpha-carotene and beta-carotene, compounds shown to work against cancer cell growth. Comparing it to a soda gets the food badly wrong.
Q: If sugar feeds cancer, why can I have carrot juice but not fruit juice?
A: Because of what comes with it, and how much. Fruit juice is concentrated sugar with the fiber stripped out and comparatively little else. Carrot juice is low-glycemic and carries a dense load of cancer-protective compounds. During active disease we take fruit juice out entirely and keep whole fruit small and infrequent, while vegetable juice stays in.
Q: How much carrot juice should someone drink?
A: Our usual recommendation is one to two eight-ounce glasses of fresh vegetable juice daily, with carrot as part of the mix rather than the whole glass, plus two servings of BarleyMax — which puts the carrot itself at about four to five ounces a day. The therapeutic protocols people read about run far higher — around forty ounces daily, from five pounds of carrots — and that is a serious undertaking best done with a nutritionally minded practitioner involved, particularly if there’s any kidney or liver concern.
Q: Is it better to juice carrots or blend them?
A: Both are good, and they trade off. Juicing delivers more than twice the beta-carotene into the blood and goes down easily when appetite is gone. Blending keeps the fiber, which steadies blood sugar and feeds the gut, and holds onto some falcarinol that a juicer discards. If you’re able to eat normally, blending is a fine choice. If you’re struggling to take anything in, juice.
Q: Should carrots be cooked or raw?
A: Raw, for this purpose. Falcarinol is destroyed by heat within about two minutes at cooking temperature — and in the Danish cohort study, cooked carrots showed no protective effect against the cancers that raw carrots protected against. Cooking does make carotenoids more available, so cooked carrots aren’t worthless. But the compounds we’re after here need the carrot raw.
Q: Will carrot juice turn my skin orange?
A: In large daily amounts, it can — it’s called carotenemia. It shows up first on the palms, the soles and the creases beside the nose, it is harmless, and it fades as intake drops, though it can take a few months because carotenoids are stored in fat. The way to tell it from jaundice is the whites of the eyes: carotenemia spares them, jaundice does not.
There is a liver connection, but it runs the other direction from what people assume. Carrots do not harm the liver. The liver is where a good deal of beta-carotene is converted into vitamin A — so when the liver is struggling, or the thyroid is underactive, or blood sugar is poorly controlled, that conversion slows and carotene backs up in the skin at ordinary intakes. If the orange cast appears without a lot of carrots behind it, that is worth looking into with a nutritionally minded health care professional. If it appears after months of daily juice, it is doing exactly what it should.
Q: Isn’t the Ann Cameron story just one person?
A: Yes, and we’d rather say so plainly than oversell it. It’s one woman’s well-documented case, not a trial — no one has run the trial. What sits behind it is a much larger body of evidence: a meta-analysis of 198 studies, a 25-year Danish cohort of nearly 56,000 people, and controlled work on how much of the carrot’s nutrition actually reaches the bloodstream from juice.
Where to Begin
If the juicer is in a cupboard somewhere, this week is a good week to get it out.
